B-ball physicals

Parents:
First off, if your child has already played a Fall sport or turned in a PIAA Sports Physical this Fall (not an academic or State physical), then I only need a Recertification Form signed by you (parent/ guardian).

• If there was an injury after the initial physical, and I do not already have a doctor’s clearance note about it, then I need the Recertification form that the appropriate Physician signs off on clearing them for full participation.

For those who are starting up this Winter then the full PIAA Sports Physical packet will need to be filled out and signed by a parent or guardian, as well as signed by a physician. If you chose to opt out of the Sports Physicals being offered at McDevitt, then a Physician will need to sign those forms before participation.

To make this process a little more convenient, we are bringing over some great sports physicians from UPMC to provide sports physicals. This one day event will be November 7th, from 9:00am – 11:30AM.
